Welcome Marie Craigen
to the Hospital@Home Program
Please join us in welcoming Marie Craigen to the PEFHT Team - she will work with the Hospital@Home team as a social services worker - working with the team to ensure that our most complex patients and their caregivers are connected with the full range of community resources and supports they need on their journey to wellness.
We are very excited to now have the full complement of the H@H team on the ground.
