About Us

What is a Family Health Team?

A Family Health Team is a group of very skilled health professionals working together to provide you with better access to high quality co-ordinated health care and health services, as close as possible to where you live. The team can help you to navigate through our complex health care system. They can also help you to learn the skills necessary to manage your health conditions. To better meet your needs, the Family Health Team uses electronic medical records, which improves access to health information, patient records and test results.

Our History  

The Prince Edward Family Health Team was incorporated in August 2006 under the governance of our family physicians. From its first modest gains of two new nurse practitioners in 2006, the Team has grown to include 34 brand new healthcare and administrative professionals supporting the 23,000 patients of the County's 23 family physicians. Services are delivered from 2 communities – Picton and Wellington.  Our team has Nurse Practitioners, Registered Nurses, Social Workers, Dietitians and a Pharmacist providing a vast array of specialized services and clinics for all rostered patients.

The Family Health Team has had particular success in recruiting specialist physicians to work from our visiting specialist clinics in Picton. County residents are now able to see specialists in the Heart Function Clinic and Cardiac Rehabilitation; Psychiatry (adult and geriatric); Neurology; Respirology; Rheumatology; Chronic Pain; Pediatrics; Orthopedic Medicine and Vascular Disease all without having to travel to Belleville, Kingston, Toronto or Ottawa.

Together our Family Physicians, Specialists and Team of Interdisciplinary Professionals bring you a number of PEFHT Specialized Clinics including:
