Memory Clinic
The Memory Clinic is a multidisciplinary team that assesses patients with memory concerns. This is done with a variety of cognitive testing and other tools with both the patient and their family. This is done with a combination of cognitive testing, interviews and other tools with both the patient and their family.
Goal:
The goal is to provide a comprehensive assessment of cognitive concerns and a diagnosis where possible. This assessment will aid the family physician in the care of the patient along with providing a framework for the patient and their care partners to help them on their journey. For many memory loss conditions, early diagnosis and treatment can help to maintain independent living and quality of life for as long as possible.
Team:
Our Memory Clinic Team comprises two family physicians, nurses, a mental health worker, a pharmacist, and a representative from the Alzheimer Society.
The Memory Clinic is held twice monthly at the PEFHT Harbourview Clinic. Each appointment will take approximately 2 - 3 hours. We strongly encourage patients to bring along a family member or someone who knows them well; this will help ensure a thorough assessment.
